- Enter your email (this should match the e-mail that you put on your registration form during your visit)
- Enter the 7 digit ID number that was given to you at your appointment (if you are unsure, please call the Health Center for assistance)
- Enter your name exactly as it was entered into our system when you made your appointment (e.g. Kathleen Smith, not Kathie Smith)
- After you submit your request, you will receive an email with a link you need to follow to create your password and access your account
